Minor Model Release Form

National Ice Cream Day Kids Portraits presented by Alice Malia Photography LLC and Humble Cow Ice Cream.

Event Date: July 20th, 2025

Location: Humble Cow Ice Cream, 4528 Maple Ln SE Suite 104, Olympia, WA 98501

This agreement is made between the undersigned parent or legal guardian (“Parent/Guardian”) of the minor child named below (“Minor”), and Alice Malia Photography LLC (“Photographer”), in collaboration with Humble Cow Ice Cream (“Event Partner”).

By participating in this event and accepting complimentary portraits, the Parent/Guardian agrees to the following:

1. Consent to Photograph

I, the undersigned Parent/Guardian, give permission for my minor child to be photographed by Alice Malia Photography LLC during the National Ice Cream Day Kids Portraits event.

2. Photo Usage & Public Display

I understand that in exchange for receiving free professional portraits, the images taken of my child may be used by Alice Malia Photography LLC and Humble Cow Ice Cream for marketing and promotional purposes. This includes (but is not limited to) use on websites, social media platforms, printed materials, and other public-facing media.

By agreeing to have my child photographed, I acknowledge and consent to this public and commercial use of the images.

3. No Compensation

I understand and agree that the portraits are provided free of charge in exchange for permission to use the photos for marketing. I will not receive monetary compensation or royalties for the use of these images.

4. Image Ownership

I acknowledge that all photographs taken remain the property of Alice Malia Photography LLC and may be edited, altered, published, or shared at the Photographer’s discretion.

5. Release of Liability

I release and discharge Alice Malia Photography LLC and Humble Cow Ice Cream from any and all claims, demands, or liabilities in connection with the use of the photographs.
